Social Prescribing Pilot Project

This Hull CCG (Clinical Commissioning Group) funded project is the vision of the Building Health Partnerships (BHP) Steering Group whose overarching aim is to deliver improved health outcomes for our community by working more effectively with our local commissioners.

Referrals are made by your GP or health practitioner as we increasingly recognise that illness cannot always be cured by medicine and tablets alone. This project aims to provide individuals with support and access to activities in the local community to enable them to improve their health and well-being.

This project is managed by NBF and harnesses the expertise of a team of skilled volunteers recruited from member organisations of the BHP Steering Group.
